Adoption Contract
I/We the undersigned ADOPTER agree to the following terms and conditions of this contract in the acquisition of the above-described dog herein referred to as DOG. This agreement will go into effect   and will continue until notification to Delilah’s 2nd Chance Rescue, Inc. herein referred to as D2C by the ADOPTER of the death of the DOG. The terms and conditions are as follows:
1. TO RETURN DOG TO D2C IF UNABLE TO KEEP AT ANY TIME. To never abandon, surrender, sell, trade, or give DOG to another party (including family, friend, shelter/animal control, or laboratory). Notify D2C immediately at (630)-294-8484 / info@delilahs2ndchance.org. ADOPTER must give D2C 1 week to find an available space for DOG and drop DOG off at the selected destination.
In the event you can no longer care for your dog and need to rehome, you must contact D2CR. If we have open fosters we would take the dog back in or case by case to assist with rehoming.
2. D2C does not guarantee the health of DOG - this includes unforeseen inherited genetic disorder/diseases, such as seizures, cataracts, hip dysplasia, etc. Upon adoption, D2C will notify the ADOPTER of any found genetic disorder/disease as diagnosed by a veterinarian
3. To never allow DOG to be used for dog fighting, trained as protection or in medical experimentation. The ADOPTER agrees not to subject DOG to abuse, cruelty or neglect of any kind.
4. The ADOPTER agrees to provide DOG with necessary veterinary care upon sickness, disease, or injury, and take DOG to a veterinarian at least once a year for an annual health examination and routine vaccinations (including rabies). D2C will contact ADOPTER’s veterinarian in 2 weeks to verify the ADOPTER has transferred the DOG’s license/rabies tag into the ADOPTER’s name.
5. DOG will be given an annual heartworm test and monthly heartworm preventative for the entire 12 months of the calendar year. Missing one dosage could put DOG at serious health risk, putting ADOPTER in violation of this Adoption Agreement.
6. In the event of a hurricane, ADOPTER agrees to take along DOG (securing DOG safely within the shelter/home/hotel room (not a car, outdoor kennel, backyard, patio), leave DOG in the care of a responsible friend/relative who knows the breed, or at a vet/kennel office. 7. The ADOPTER agrees not to transport DOG in an open vehicle (ie: an uncapped truck bed or open jeep type vehicle). In addition, DOG shall not be left unattended in any vehicle (running or not, windows open or not) in any type of weather or for any amount of time.
8. ADOPTER agrees to respond/reply to phone calls/emails/letters from D2C Board Members - or Members appointed by the D2C Board - within a timely manner; preferably within 24 hours.
9. The ADOPTER agrees to let D2C check on DOG periodically by phone, email or personal home visit.
10. The ADOPTER agrees to contact D2C or an obedience trainer/behaviorist should behavioral problems arise with DOG. Instruction should be praise-based with positive reinforcement. Off leash training is NOT allowed as it is considered as a threat for the safety of DOG; therefore D2C will uphold this contract and reserves the right to remove DOG from ADOPTER’S home.
11.You must update microchip to your information in a timely matter, no longer than 3 months from time of adoption
Should the ADOPTER breach any of the agreements contained in this contract; D2C has the right to rescind this Contract. For legal purposes, the ADOPTER agrees that the declared value of this dog is no less than one thousand dollars ($1000.00). The ADOPTER recognizes that non-compliance with any of the above terms can result in forfeiture of the above named DOG to D2C without payment of compensation. The ADOPTER also acknowledges that D2C has advised the ADOPTER that DOG may have been neglected or abused and may require special care or precautions. The ADOPTER is also aware that DOG may dig, chew, soil or exhibit other undesirable traits unless properly supervised and trained. In consideration to the adoption of this DOG, the ADOPTER agrees to assume full responsibility for the conduct of DOG upon the date of this contract. The ADOPTER agrees to hold harmless D2C its board, officers, directors, volunteers and any individual or affiliate involved with D2C for any damages or costs resulting from liability, known or unknown, anticipated or unanticipated as a result of DOG’s conduct on or after the date of this adoption contract on .
*Only Optional Donations are tax deductible: According to the State of Florida, Donors may only deduct contributions to our organization to the extent that their contributions are gifts, with no consideration received. 100% of all donations go towards the D2C rescue dogs.
I accept that all fees are not refundable. If the animal was not given sufficient time to decompress, Delilah's 2nd Chance typically use these funds to re-evaluate the animal. Sometimes veterinarian visits and or/behavior assessments via experts are needed. Delilah's 2nd Chance's advice is to be followed regarding training (we provide a free training session with all adoptions), products, medication, and time given to the animal to adjust to their new home and family.
